The Limits of Real-Time Eligibility

The electronic transaction confirms coverage exists. It frequently cannot confirm benefit detail, which is why automation reduces but never eliminates payer calls.

Electronic eligibility verification is fast, cheap, and batchable. It is also more limited than the people relying on it usually realize.

What it reliably returns

Whether a plan is active on a date, the plan identifiers, and often basic coverage categories. That is genuinely useful and it catches the largest category of front-end failure — patients whose coverage has lapsed.

What it frequently does not

  • Deductible met to date, which is the number patients most want and the one most often stale or absent.
  • Service-specific coverage. Active coverage does not mean this procedure is covered.
  • Authorization requirements, which are rarely returned reliably and are the most expensive thing to get wrong.
  • Visit or unit limits, and how many have been used.
  • Network status for this specific provider under this specific plan variant.

Payers differ enormously in what they return, and a response that omits a field is not the same as a field being zero.

The practical consequence

Automation reduces payer calls; it does not remove them. Any workflow promising full verification with no human contact is describing eligibility checking and calling it benefits verification.

Where to spend the calls

Triage. Routine visits on familiar plans need the electronic check only. High-cost procedures, new patients, high-deductible plans, and anything that might require authorization justify the call. That is a rule staff can apply consistently, and it puts the expensive step where the money is.
